Hospitalization Procedures

Hospitalization is advised by a doctor after an outpatient consultation.

1. Issuing of a Hospitalization Reservation Slip (after an outpatient consultation)

  • œA doctor provides you with a hospitalization reservation slip. Go to the admission counter and fill out the application. Consult with the medical affairs section about your hospitalization fee in advance if necessary.
  • œWe will call you later about the date of admission. Remember that you might have to wait for a while due to limited space.

2. Admission

3. Procedure of the Admission Day

Submit the following documents at the admission counter on the day of admission.

  • œNecessary Documents
    • 1.Hospital Registration Card
    • 2.Health Insurance Card
      You must pay the full amount of the bill if the submission of your health insurance is late. Go to the admission counter if there are any changes to your health insurance card or if you lose eligibility.
    • 3.Hospitalization Guarantee Form
      Read everything carefully and ask your joint guarantor (an adult of a separate household) to sign and stamp on the form after his/her confirming the information.
    • 4.Hospitalization History Form
      If you have been hospitalized in other hospitals in the past three months, write the periods. Submit certificates of hospital discharge (if you have any).
  • œCarts are available at the entrance of the outpatient ward to transport your luggage. Please return after use.
  • œIf you are hospitalized due to a traffic accident, tell a personnel at the admission counter.
    Ask at the office of insurance and welfare (on the 1st floor of the outpatient ward) for details of medical expenses paid by the government (Kouhi Futan Iryou).
