Hospitalization and Discharge Procedures

Pre-Hospitalization

1. Issuing of a Hospitalization Reservation Slip

A doctor provides you with a hospitalization reservation slip.
Go to the “Patient Support Center” at Outpatient Ward 1F and fill out the application.

  • * If you are worried about paying hospitalization fees, please consult with staff of Reception for hospitalization and discharge in advance.
  • * For payment methods, please contact Income and Payment Management Section, Medical Services Division.

2. Admission

Please come to reception below.

3. The date of admission

We will call you about the date of admission after the hospitalization date determined.
Remember that you might have to wait for a while due to limited space.

4. Preparation

Prepare the items written in “What to bring”.

Hospitalization

1. Necessary Documents

●Hospital Registration Card

●Health Insurance Card

If the submission of your health insurance is late, you must pay the full amount of the bill.
Go to the admission counter if there are any changes to your health insurance card or if you lose eligibility.

●Hospitalization Guarantee Form

Read everything carefully and ask your joint guarantor (an adult of a separate household) to sign and stamp on the form after his/her confirming the information.

●Hospitalization History Form

If you have been hospitalized in other hospitals within three months, write the periods and submit certificates of hospital discharge (if you have).

2. Admission (on the day of admission)

Submit the above documents at the admission counter on the day of admission.

  • *If you are hospitalized due to a traffic accident, tell a personnel at the admission counter.

●Parking

Parking is free on the day of admission and discharge.
Please go to the following admission counter to make the parking fee free.

  • *Security guard box is across from the off-hours reception.

●Luggage cart

Luggage carts are available at the entrance of the outpatient ward.
Please return after use.

●Traffic accident

If you are hospitalized due to a traffic accident, you need to tell staff at above reception on the day of hospitalization.

●Publicly funded healthcare

Ask at the office of insurance and welfare (Patient Support Center) for details of Publicly funded healthcare, “Kouhi Futan Iryou”.

Discharge

1. Permission of discharge

The date of discharge is determined in consultation with a doctor.
If you will make outpatient visits after discharge, please receive the reservation slip.

2. Payment

We will give you the bill on the day of discharge or by mail later.
