Consultation Guide

All clinical departments request “Referral Letter” from other medical institution for new patients.
Without an appointment, you may have to wait a long time.
Making an Appointment

For Outpatients and Their Attendants

When visiting, please note the following:

  • We do not provide vaccination and outpatient consultation for COVID-19.
  • ●Please keep the number of attendants to a minimum.
  • ●Please notify our office staff if you have any symptoms such as fever, cough, sore throat, or any abnormalities in your sense of taste or smell.
  • ●All individuals must wear a surgical mask at all times throughout the hospital.
  • ●Please refrain as much as possible from bringing accompanying persons into a consultation room.

Triage (Screening for COVID-19 symptoms)

  • ●Outpatients and their attendants are asked to check their body temperature using a thermometer to confirm their health status.
  • ●Depending on the temperature and physical condition, a doctor or nurse may conduct a further interview.

What to bring

●Japanese Health Insurance Card

If you do not have or did not bring your health insurance card, you will be charged with the full amount of the bill.

  • ・When there are any changes to your health insurance card, such as renewals of duration
    *Please inform the reception desk if you have any changes.

●Patient registration card (if you have)

Please bring your patient registration card for every visit.
It is valid for all the clinical departments.

●Referral Letter (New patients, Patients who revisit our hospital after more than six months)

Patients CANNOT consult any clinical department without referral letter.

    Submit “reservation confirmation document”, “health insurance card” and “referral letter” to New Patients Reception (no.2) on the 1st floor of the Outpatient Ward.
  • ●Without appointment
    • 1. Come to New Patients Reception(no.2) and pick up a numbered ticket to wait for your turn.
    • 2. Fill out “application form” for new patients near New Patients Reception(no.2).
    • 3. When your number is called, Submit “application form”, “health insurance card” and “referral letter” to New Patients Reception (no.2).
    • * All clinical departments request “referral letter” from other medical institution for new patients.

Revisit patients

  • ●With appointment by oneself
  • ●Without appointment
    • 1. Come to Revisit Patients Reception(no.4) and pick up a numbered ticket to wait for your turn.
    • 2. Fill out “application form” for revisit patients near Revisit Patients Reception(no.4).
    • 3. When your number is called, Submit “application form”, “health insurance card” and “referral letter” to New Patients Reception (no.4).
    • * All clinical departments request “referral letter” only for patients who revisit our hospital after more than six months or who want to consult another clinical department of our hospital.

2. Consultation/Examination

Please follow the instructions by doctors and nurses at each department.

  • *Due to the nature of medical services, please understand that delays do occasionally occur.

3. Billing (Outpatient Ward 1F)

Go to the reception counter (no.5).

4. Payment (Outpatient Ward 1F)

Go to the reception counter (no.20)

  • *Payment at the counter takes longer. If you are in a rush, use the automated payment machines. There are two types of payment machines; cash only and credit / debit cards only.

5. Patients with Prescriptions

●In-house Prescriptions (Outpatient Ward B1F)

Go to the Clinical Pharmacology and Therapeutics in the Hospital.

●External Prescriptions

Go to an External Clinical Pharmacology and Therapeutics.
